Frequently Asked Questions

  • "Food is Medicine" is a philosophy that suggests that food and nutrition can play a key role in sustaining health and preventing disease - and be used as a therapy for chronic diseases that are responsive to changes in diet - helping to improve symptoms, slow disease progression, and improve quality of life.

  • Once members are enrolled in the program, they will complete an intake assessment and schedule their first nutrition coaching session with a NourishedRx Registered Dietitian. The member will receive their first healthy home-delivered food after completing their intake assessment, and will continue to receive food for 4 months. The member will engage in nutrition coaching sessions with their NourishedRx Registered Dietitian and will have additional support from a NourishedRx wellness associate for 6 months.

  • To be eligible to participate in the program, members must be actively enrolled in an eligible Blue Cross NC health plan, age 18 and older, diagnosed with type 2 diabetes, have an HbA1c value of 8.0% or greater in the last six months, and not meet any of the following exclusion criteria: liquid diet, stage 5 chronic kidney disease, end-stage renal disease, renal replacement or dialysis, enrolled in hospice, or living in a nursing or long term care facility.

  • The primary objective of the program is to reduce HbA1c levels by at least 1.0%, and ensure members maintain that reduction for the long term. Reducing HbA1c levels slows the progression of diabetes, reduces the risk of complications, reduces healthcare utilization, and reduces the need for long-term diabetes medications. Additional program impacts include improvement in nutrition security, improved confidence and knowledge in managing diabetes and a healthy diet, improved physical and mental quality of life, and reduced distress in managing life with diabetes.
